LOCATION: Tucson Hospitality House -1002 N. Main Ave
STATUS: 40 Hours Full-Time - Sunday through Thursday (Friday and Saturday off)

QUALIFICATIONS:
- Associates degree in a Human Services or related field, or minimum two years of community social services experience may be substituted for degree requirements.
- Experience working with clients in crisis and a strong knowledge of the cycles that affect at-risk individuals experiencing homelessness.
- Knowledge of community resources, particularly housing for low-income individuals
- Knowledge of Homeless Management Information System
- CPR/First Aid certification or willingness to obtain it.
- Computer literate in MS Office Software
- Ability to work with people of diverse backgrounds and circumstances
- Ability to assess at risk participants and situations
- Comprehensive knowledge of local community resources
- Excellent interpersonal communication skills, both written and verbal
- Successful background clearance
- Must possess a valid Arizona driver’s license and pass a motor vehicle record check.
RESPONSIBILITIES:
- Conduct interview, assess immediate short and long-term needs, develop case plans including budgets, expenditure accounting, referrals, and target goals, providing appropriate resources and tools
- Learn and maintain knowledge of community employment agencies, other resources for gainful employment, help alleviate housing and employment barriers.
- Provide case management and needed services to individuals in compliance with program policies and procedures
- Assist clients to identify mainstream needs and facilitate the process with appropriate service providers to meet those needs.
- Complete detailed files in both HMIS and hard copy file, with accurate and concise case note, goals for all clients assisted, including profiles, social history, entering services and referrals in HMIS
- Provide clients and public households with application procedures, eligibility requirements and benefits of SNAP/AHCCCS programs
- Assist same clients with tracking their case through Health-e-Arizona and DES phone contact when needed
- Assist with completion of Intake/Assessment, V1-SPADAT documents as required daily for all clients, per AHP Grant compliance and Federal Home Loan Bank
- Provide nightly outreach services for new homeless clients
- Help residents resolve issues related to the community living in conjunction with the shelter manager.
- Report issues of concern, abuse, or neglect to Program Manager immediately
- Perform follow up communication with referral agencies as needed
- Represent The Salvation Army when interfacing with social service agencies, committees and other groups as directed
- When assigned, act as liaison with outside agencies to disseminate information regarding for the homeless services that are provided by The Salvation Army
- Set correct Transitional fees for programs participants being certain to maintain the integrity of the fee program
- Transport clients to appointments as necessary utilizing TSA vehicles
- Attending all staff meetings and all required training.
- Other duties as assigned by supervisor as it relates to the position of Case Manager
PHYSICAL REQUIREMENTS:
While performing the duties of this job, the employee is regularly required to walk, stand, climb or balance, stoop, kneel, crouch, crawl and reach with hands and arms. The employee must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 35 pounds, and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to adjust focus.
